Important Tax Notice: Request Your 2025 Health Coverage Form (1095-C or 1095-B)

Under IRS guidance (Notice 2025-2015), CT&T Enterprises, Inc. is not required to automatically mail Form 1095-C or 1095-B to all individuals. However, you have the right to request a copy of your Form, which reports the health coverage you were offered or enrolled in during the 2025 calendar year.

 

How to Request Your Form

 

To request a copy of your 2025 Form 1095-C or 1095-B, please contact us using one of the following methods:

What You Need to Know

 

• Your request must include your full name, mailing address, and last four digits of your SSN.

• Once we receive your request, we will provide your form within 30 days or by January 31, 2026, whichever is later.

• If you consent, we can send your form electronically.

 

This notice will remain available on our website through October 15, 2026.
